Provisional That's some pretty bad putting, my friend.
Do you sway, shift weight, rotate, break the wrists, "hit" the ball?
Take a phone charger and hold it in your hands with the putter, with the heavy end (the part that plugs into the wall) dangling at least a couple of feet toward the floor. Take practice swings with the putter and do it so that the putter and the dangling cord travel together. If you jerk quickly on the backswing or move forward too quickly on the foreward swing the cord with not travel with the club, and you can identify the problem. Practice swinging the putter back and forth in perfect rhythm with the cord. Hold your body completely still, without any movement whatsoever except in the shoulders and arms. Do this exercise without a golf ball. Just feel the rhythm and try to internalize it.
Next, do you have a carpet in your house that would be reasonable for putting practice during the winter? If so, get three balls, put one out as a target, and roll the others at the first one. The ball is much smaller than a golf hole so it focuses you better. Try short distances first and then expand to slightly longer putts. Try to keep the rhythm you learned in the first exercise as you roll the ball.
