I usually make one read at about 2/3rds of the way to the hole. If I feel the need to hold up fingers when I get to the ball, then I do. More often I know what the fingers will show. So none of that takes long. Really all I want is the direction and rough slope.
I don’t expect it to be much help on a double breaker. But I guess it can be done. I don’t see enough of those to bother, and they are usually obvious besides. I don’t get many multi tiers either, but I would probably try to read the last part of the putt with Aimpoint.
You don’t have to straddle the line if you’d rather stand off to the downhill side. Obviously the actual line is the place you want but whatever. It’s not hard to straddle a line.
Up or downhill, I’d say it’s up to the user to understand how speed affects break. A more advanced user will vary the distance of the fingers from their eyes according to the speed of the putt. Faster putt, hold the fingers a little closer to your eyes. I’m not into it enough to mess with that.
